• To apply for this scheme, eligible beneficiaries need to collect the application form from their Urban/Rural Block Office or District Collector Office. The application form is available free of Cost.
• Have to fill this application form with the correct information.

• After completing the form, submit the attached documents in the same office.
• You can also send your application form by post.
• After receiving the Application Form, you will get a receipt for the same.
